Additional Benefits:

MEALS: House Staff receives free meals while on duty.

LAB COATS: Residents receive two lab coats per year.

PROFESSIONAL LIABILITY INSURANCE:  The hospital obtains and pays for malpractice insurance.

MEDICAL AND PRESCRIPTION BENEFITS:  The hospital provides three choices of medical insurance plans for residents and their immediate family.   Specific information, including employee contribution rates per pay period, is available in the Human Resource department.  This coverage is effective the first of the month after 30 days of employment.  A pharmacy benefit is automatically included when you enroll for one of the hospital’s medical insurance plan.  The hospital provides reimbursement of COBRA premiums paid during the first 30 days of employment.

DENTAL BENEFIT:  Available through the hospital sponsored benefit program at discounted group rates.

VISION BENEFIT:  Available through the hospital sponsored benefit program at discounted group rates.

HEALTH CLUB MEMBERSHIP:  $200 toward membership fee at any fitness facility of choice.  Exercise rooms available within hospital for house staff use.

LIFE AND ACCIDENTAL DEATH & DISMEMBERMENT INSURANCE:  Life insurance coverage of one time your salary is provided by Affinity Medical Center; Optional Life Insurance up to 5 times your salary also offered at employee expense.    Dependent life and AD&D Insurance from $5,000 per dependent to $10,000 each child and $25,000 for spouse available to purchase.  Voluntary AD&D available from $25,000 to $500,000 at employee’s expense.

DISABILITY INSURANCE:  Short Term Disability is a voluntary purchase, with coverage up to 60% of weekly pay after 30 days or after EIB benefits have been exhausted, whichever is later. Long Term Disability is also a voluntary purchase, with coverage up to 60% of monthly pay after 150 days of non-occupational disability.

OHIO TRAINING CERTIFICATE/LICENSURE:  Temporary Ohio Training Certificate fees are paid by hospital for resident OGME 1.  Residents OGME 2-5 must obtain a permanent license in the State of Ohio and may use their educational stipend for this expense.

PART III BOARD/INSERVICE EXAMS:  Fees paid by the hospital for residents.

MEDICAL LIBRARY: Residents have 24-7 access the the medical library, which has a part-time medical Iibrarian to assist in your research and studying needs.

HEPATITIS B VACCINE:  This is provided, at hospital expense, to all resident as well as annual Flu vaccinations and annual TB tests.

MOVING EXPENSES:  Maximum of $500 based on actual expense. One time allowance at start of program.

SCHOLARSHIP OPTIONS:  Service League for Community Health and Education.
